In-School Flu Vaccinations

MPS Health Services is collaborating with Smiles by Rosie to provide in-school flu vaccination clinics for students. Please see attached forms. 

Please return consent forms to the school nurses by October 4 so that we can account for the appropriate number of vaccines and schedule the clinics per school.
